While getting a passport issued under your name, there are a few documents that need to be submitted. First after paying the relevant fees of Rs. 1500 or Rs. 1000 for a 36 page passport booklet or a 60 page passport booklet respectively, the application form is given to the passport seeker. The application form has to be duly filled in legible hand writing. The application form may be got from the local passport office or can even be downloaded from the Net.
The proof of Residence has to be submitted. Now as a proof of Residence, the Voter’s ID, Ration Card, Electricity Bill, Telephone Bill, Bank Account Statement or the Income Tax statement may be used. Two self attested copies of this Residence proof are required.
Next, the date of birth proof is required. For which two copies of the Birth Certificate may be submitted. If you actually haven’t stayed at that particular residence for the past one year, then you may need to present an additional form. This form is known as the “Personal Particulars Form” which has the details of the other residence in which the said person was staying. This form has to be supplemented with copies of relevant documents proving his claim.
If your name has been changed now and is different from the one that appears on the Birth Certificate, then you need to submit either your husbands passport copy, if the change in the name is because of marriage or an affidavit stating that the name has been changed which has to be supplemented with a copy of the print out of the Newspaper where in the change of name had been printed.
